Ketamine Costs & Spravato Coverage in Tennessee

Ketamine and esketamine are not interchangeable for insurance purposes. Many off-label ketamine programs are self-pay. Spravato coverage depends on your individual plan, eligibility and authorization; approval is never guaranteed.

Before you book treatment

Ask for a written estimate

IV, IM and compounded ketamine for psychiatric conditions are off-label; many programs are self-pay. Ask about assessment, medication, administration, monitoring, therapy and maintenance charges separately. We do not assume a statewide price.

Check Spravato coverage

Spravato is FDA-approved esketamine, not the same as ketamine infusions. Coverage is not guaranteed. Check eligibility, prior authorization, network status, drug and visit charges, deductibles and copays with your insurer and clinic.

Plan a ride & follow-up

Spravato requires a certified healthcare setting and monitoring for at least two hours. Arrange transport home; do not drive until the next day after restful sleep. For other ketamine treatments, follow your clinician's instructions. Follow-up and maintenance depend on response and tolerability.

Ask who manages your medications, how your usual clinician will receive updates, and whom to contact between visits. Neither a shortage designation nor local depression statistics show that ketamine is appropriate or effective for you. Consult a licensed physician.

Questions for your clinic and insurer

  • Ask whether the exact treatment and diagnosis are covered—not just whether the clinic accepts your insurer.
  • Confirm the treating office, prescriber and medication are in network and which charges count toward your deductible.
  • Request current prior-authorization requirements and provide your clinician with your medication and treatment history.
  • Ask for separate estimates for evaluation, medication, administration, observation, therapy and maintenance visits.
  • If authorization is denied, request the written reason and ask your insurer and clinician about the applicable appeal process.
